You'll receive complimentary preventive services and much more!

A premium payment of $389.00 for a full year includes 100% discount on the following services:

• 2 Periodic exams
• 2 Routine cleanings*
• 2 Fluoride treatments
• 1 Set of x-rays
• 2 Oral cancer screenings

*Periodontal maintenance visits will be discounted at 5O%. This applies to patients who have had treatment for gum disease.

You'll also receive 20% discount on ALL other services including fillings, crowns and night guard appliances!

Our Dental Savings Plan is designed as a discount plan- not an insurance plan. It cannot be used in conjunction with dental insurance. You may not combine it with any other discounts that are already being offered. The plan is subject to the following rules and guidelines:

• Valid only for services performed in our office
• Not valid for services covered by Worker's Compensation
• Premium is non-refundable and not eligible for transfer to another patient
• Amounts not used during eligibility periods are non-refundable and cannot be used in another benefit period.
• The patient's portion of the bill is due in full at the time of service.*
